How to register:
Please stop in the district administration office for a registration packet.
Summer hours: Monday-Thursday 8:00-4:00 CLOSED on FRIDAYS
To finalize registration at Lakeview School District, please bring the packet to the District Administration office along with the following other documents:
Completed packet
Proof of Residency (see form in the packet)
Birth Certificate (we can make a copy of the original)
Updated Immunization Record
Name, address and phone number of previous school
Court orders concerning custody issues
