Have you moved recently and need to inform the school?
The District of Primary Residence Certification below is the form you need to complete and send to the district office in order to make the change. Additionally, all of the supporting documents as listed on the form must be submitted with the completed form.
Do you need to change your student's transportation pick up or drop off location?
The Transportation Request Form Change below is the form you need to complete and send to the district office in order to make the change.
If you are requesting a transportation change due to a residence change, both the District of Primary Residence Certification with supporting documents and Transportation Request Form Change must be completed and submitted to the district office.
